Bilingual (Spanish) Care Manager

Description:Working under the direction of the department manager, educates program participants on the most current health information for the management of their chronic condition. Documents all encounter data for reporting purposes. Emerging member risk will be identified for medical intervention using clinical protocols established under national guidelines.


• keep informed of drug recalls, new medications, studies, etc. and provide daily updates to clinical pharmacy team when requested
• learn data systems required to log calls, research claims, or other member related information
• become familiar with PHM implementations, plan designs and formularies
• place calls to enrolled participants on assigned days, develop and maintain relationships with these members
• attend team meetings and health-focused educational sessions weekly
• run queries, research and compile data from various sources to assist in the development of documents, reports, studies, etc.
• create, revise and maintain PHM related documents, spreadsheets and binders upon request
• communicate with pharmacies and physician’s offices regarding member claims, co-pays, labs, etc.
• provide PHM Operations Manager with summary of completed tasks and future goals on weekly basis
• assist clinical team with additional projects upon request

• strong verbal and written communication skills in English and Spanish
• ability to adapt to change
• ability to handle multiple tasks
• strong focus on work quality
• intermediate computer skills to include Microsoft Excel, Word, Outlook and internet search engines
• capability of working independently or as part of a team in a friendly corporate environment


• associates degree preferred
• license as pharmacy technician or medical assistant preferred (licensure required within first 6 months of employment)
• knowledge of medical terminology
